high. Only distal muscular responses would be accepted. We would be interested to know more about patient acceptance of the procedure, as penetration of the pectoral muscle with a thin needle is sometimes painful. What about a large 17 or 18G Tuohy needle? Although sophisticated methods may be helpful in some circumstances, we believe no device will ever replace good training in the ®eld of peripheral nerve block.

Sub-Tenon's infiltration using bupivacaine 0.5% decreases acute postoperative pain.
EditorÐWe would like to thank Calenda and colleagues for contributing the results of their trial to the ongoing controversy over the use of sub-Tenon's block (STB) in posterior segment eye surgery. The use of STB continues to expand into all areas of ophthalmic surgery. A good evidence base for the choice of anaesthetic technique even in cataract surgery is unfortunately missing. The lack of go...
متن کاملEffect of Incisional Site Infiltration of Bupiva-caine on Postoperative Pain and Meperidine Consumption after Midline Laparotomy
The purpose of the current study was to determine whether infiltration of bupivacaine in the incision site of midline laparotomy reduces postoperative pain and opioid consumption. Fifty-six, 30-60 year-old patients who were undergoing midline laparotomy were enrolled in the present study. The patients were randomly assigned into two groups of control (group C, n = 28) or bupivacaine (group B, n...
متن کاملRandomized double-blind clinical trial comparing topical and sub-Tenon's anaesthesia in routine cataract surgery.
BACKGROUND Several local anaesthetic techniques are available for cataract surgery. Recently, topical anaesthesia has gained in popularity. A randomized trial was designed to compare patient discomfort and intraoperative complications following routine cataract surgery under topical or sub-Tenon's anaesthesia. METHODS A randomized double-blinded placebo-controlled clinical trial of 210 patien...
متن کاملThe Effect of Sub-Tenon's Bupivacaine on Oculocardiac Reflex during Strabismus Surgery and Postoperative Pain: A Randomized Clinical Trial
PURPOSE Oculocardiac reflex (OCR), defined as bradycardia induced by manipulation of extraocular muscles, is a serious complication during strabismus surgery for which prevention and proper management is required. In the present study, we investigated the efficacy of sub-Tenon injection of bupivacaine for prevention of OCR and postoperative pain. METHODS A prospective randomized controlled cl...
متن کاملNot for Distribution
Background: The use of large doses of opioid analgesics to treat pain after cardiac surgery can prolong the time to tracheal extubation and interfere with recovery of bowel and bladder function in the postoperative period. Therefore, the authors investigated the efficacy of a continuous infusion of bupivacaine 0.25% or 0.5%, at the median sternotomy site, for 48 h after cardiac surgery in reduc...
